Otherwise, I am not an in-network provider for other major medical insurance companies and you will want to submit claims to your insurance company and use your out-of-network benefits, if applicable.

I am currently working to change this so that you may utilize your insurance benefits for my services.  However, in the mean time you may want to consider asking these questions to your insurance company prior to making an appointment:

• Do I have any out-of-network benefits for individual and/or family therapy?
• What is my deductible and has it been met?
• When does my insurance year begin, is it the calendar year and how many
  sessions per year does my plan cover?
• What is the reimbursement rate for therapy sessions for an out-of-network
  provider?
• Do I need any pre-authorization to see an out-of-network provider?
  If so provide your insurance company with my name and date of first session

Many people, for privacy reasons, choose to not involve their insurance carriers or prefer to pay out of pocket.  In those instances, my out of pocket fees are as follows:

• Individual/Family Session (45-50 minutes): $150

Payment
Cash, check and credit cards are accepted for payment.

Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay a cancellation fee in the amount of the scheduled session.
